Supporting a summit to advance adoption of super-utilizer models nationwide

This grant supports a one-day summit to advance the adoption of high-value models of care for patients with complex physical, behavioral and social needs who experience high numbers of avoidable emergency department visits and avoidable hospital readmissions (super-utilizers). A number of states, health care systems and other organizations have developed and implemented cutting-edge, super-utilizer models, including models for low-income and Medicaid patients, and many of these initiatives now have useful lessons to share. The Center for Health Care Strategies (CHCS) will design, convene and facilitate this summit, which will focus on engaging participants in sharing best practices and lessons learned from their experiences in implementing super-utilizer initiatives. Summit participants will include federal and state leaders, policymakers, Medicaid programs, health plans, and health and social service providers. Deliverables will include: a meeting report with key lessons learned, and recommendations to governors, state agencies, payers and policymakers for designing and implementing super-utilizer programs, as well as an addendum of sample legislative and/or regulatory language for states to foster their own super-utilizer initiatives. Atlantic Philanthropies has requested a proposal from CHCS to cover the staff costs associated with designing and running the summit and developing the recommendations and report. This grant will cover all meeting and report-related expenses.
