Assessing New Jersey's readiness to implement the Reclaiming Futures program model

The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.Reclaiming Futures' national program office (RF-NPO) will conduct a six-month readiness assessment for implementing Reclaiming Futures in New Jersey. During the assessment, the acceptance of the initiative by New Jersey stakeholders will be determined. Reclaiming Futures will develop a stakeholder map, hold meetings with potential state champions, and conduct assessments of local sites' readiness. This will include: (1) an overview and broad vision of system of care; (2) leadership for change; (3) planning for collaboration; (4) community-directed engagement; (5) service coordination; (6) broadening community supports; and (7) sustaining and expanding the effort. RF-NPO will also make a recommendation to New Jersey Health Initiatives on how many sites are ready for implementation of the model in New Jersey.
