This grant supports the evaluation of Improving Management of Health Care Super Utilizers, an initiative within the Aligning Forces for Quality (AF4Q) program that will adapt and pilot test a prominent model for improving care for very high users of health care and reducing costs. The evaluation will document the experiences of AF4Q communities as they adapt the model, assess how patient needs are addressed through new care-management strategies, and document changes in the use of hospital resources by patients in the initiative. Deliverables will include four reports that will address lessons learned, patient-care experiences and changes in health care utilization.
