Supporting a conference to develop integrated systems of care for dual eligibles

This grant supports a one-day convening of experts and consumers aimed at strengthening the consumer voice in delivery and payment issues in health care for dual eligibles (people eligible for both Medicare and Medicaid). The Centers for Medicare and Medicaid Services has launched demonstration projects aimed at supporting states in developing innovative ways to integrate care for dual eligibles. The planning and implementation of these new integrated delivery systems for health care must include the voices of consumers to improve the projects' chances of success. To be most effective, consumers need increased expertise on complex policies related to models of care and payment options and on developing relationships with state and federal officials, providers, and health plans on the key issues. The primary audience for this convening will be advocates, consumers, and innovative providers and funders from 10 to 12 states identified by Community Catalyst as having the greatest potential to be leaders in developing integrated systems of care for dual eligibles. The key deliverables will be increased consumer expertise on delivery and payment issues related to integrated systems of care for dual eligibles; strengthened consumer leadership skills in developing, monitoring and implementing dual-eligible initiatives in consumers' home states; development of connections between state- and federal-level advocates to support coordination of advocacy efforts; and the launch of a durable learning community of attendees.
