The Robert Wood Johnson Foundation Community Health Leaders program (CHL) was designed to provide recognition for the contributions community health leaders make to achieving RWJF's mission and goals and to enhance the capacity of these individuals to have a more permanent and widespread impact on health care problems.The mission of CHL is to support and sustain the capacity of individuals who demonstrate courage, creativity, and commitment to improve health outcomes at the community level, with a special focus on underserved populations. Toachieve the goal and carry out the mission the National Program Office (NPO) conducts initiatives to bring visibility to the 187 leaders and their work, organizes and supports opportunities for leaders to learn new skills and develop new resources, and disseminates "lessons learned" unique to the individual leaders and the criteria that define the award. This grant supports continued technical assistance and direction for the CHL program.
