Leveraging the Camden Coalition of Healthcare Providers model to improve quality and value in treatment of health care high utilizers
This grant will leverage earlier Robert Wood Johnson Foundation (RWJF) investments in improving the quality and value of care for vulnerable patients who are high health care system users (superutilizers). Jeffrey C. Brenner, MD, and his Camden Coalition of Healthcare Providers team will share expertise and resources developed through their earlier work with selected Aligning Forces for Quality (AF4Q) communities to enable them to develop local approaches to care for their community's superutilizers. Aligning Forces is RWJF's signature effort to lift the overall quality of health care in targeted communities, reduce racial and ethnic disparities, and provide models for national reform. Brenner and his team will integrate the lessons learned from earlier superutilizer work and from the AF4Q communities into a program guide intended to support other communities in developing similar local programs. The key deliverables will be: (1) funding and technical assistance to develop local superutilizer pilots in up to six Aligning Forces communities; and (2) a program guide to enable other communities to create and manage similar programs.
