Funds from this grant support Health Leads in key capacity investments to increase its scale and yield faster, more-effective, sustainable growth. Health Leads is a replicable service model that mobilizes college students to work in community health clinics to address the social factors that negatively impact the health and health care of low-income families. This model helps patients gain access to critical resources that impact their health and health care, such as food, housing, child care, and employment. It works to increase the capacity of clinics to address the needs of the whole family in prevention and treatment and, thereby, increase clinics' efficiency and effectiveness. Finally, it provides exposure to and training in the importance of social determinants of health at an impressionable time in the career of future health leaders. In this second stage of funding, Health Leads will: (1) formalize its business model; (2) expand its cohort of partner clinics to test its business model and increase earned revenue; and (3) build organizational capacity to prepare for growth. Deliverables will include: (1) a value proposition and sales strategy for Health Leads' services; (2) 10 new Health Leads sites; and (3) a sustainable growth model for third-stage funding.
