The Foundation's initiative, Aligning Forces for Quality: The Regional Market Place was designed to help communities across the country align health care forces to improve the quality of health care. The Oregon Health Care Quality Corporation's Regional Equity Action Agenda 2.0 AF4Q MATCH initiative will lead multiple partners to map the intersection of chronic disease prevalence data and data on the social, economic, and physical determinants of health for the Portland metro region. This Regional Equity Atlas 2.0 will be a powerful tool for analyzing and addressing the connections among the physical environment, health behaviors, and health outcomes for community-based groups, shape public policies, land use and transportation plans, community development projects, and investment decisions. Deliverables include the Regional Equity Atlas itself, which will be used to support policies that promote physical activity and transit use, inform policy makers on the importance of social, economic and physical determinants of health, incorporate health equity into climate change action plans, and prevent a freeway expansion in the area.
