This project--the third phase of the OpenNotes project--will report the results of the OpenNotes study; expand the use of open notes in the three study sites to more physicians and patients and develop useful tools for other institutions that wish to adopt the practice; and expand communications and advocacy efforts to popularize the idea of open notes. In addition, the project team will pilot a study to understand patients' experiences in attempting to obtain copies of their medical records (of which the notes that primary care physicians write about patients' office visits are a subset). Deliverables will include academic publications, presentations and other materials designed to help others implement open notes. Another deliverable will be a public-use dataset for inclusion in the Robert Wood Johnson Foundation's Health and Medical Care Archive.
