The Foundation's initiative, Mobilizing Action Toward Community Health (MATCH), was designed to help communities translate the County Health Rankings into multisector action that addresses the social, economic, environmental and behavioral factors that affect health.This contract provides support for planning activities for the MATCH initiative. RWJF staff will work with members of the MATCH coordinating center (the University of Wisconsin Population Health Institute and Community Catalyst) to outline and refine an evaluation plan, core set of evaluation questions, and logic model that will guide the broader evaluation of the initiative. To inform the work in the planning phase, the evaluator will review documents and interview key stakeholders at the Foundation, the Population Health Institute and Community Catalyst, and in the broader field. Deliverables will be evaluation questions, an evaluation plan, a refined logic model and a workplan.
