New Jersey Health Initiatives: Collaborating for Community Needs Assessment--United Way of Greater Mercer County

The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.Greater Mercer Public Health Partnership is a collaboration of public health departments, health care providers and community organizations. The Partnership is led by United Way of Greater Mercer County, a community service organization with over 70 years of experience in bringing groups together to measurably improve the lives of area residents. Recent changes to the health care landscape, resulting from the relocation of Capital Health-Mercer to Hopewell, and University Medical Center at Princeton to Plainsboro, create a need to review the organization and delivery of services in relationship to the community needs and health risks. While this grant's focus is on four geographic areas outside Trenton, another application was submitted that will focus exclusively on Trenton. Together these studies and the collaboration among all county stakeholders will ensure the needs of the entire county are comprehensively addressed. The needs assessment will collect quantitative and qualitative data about health needs, resources and assets, and will engage the community through interviews, surveys, focus groups and community forums. All phases of the process will be evaluated. At the end of the assessment a regional health improvement plan will be created and widely distributed. It will seek to fulfill needs and reduce duplicative efforts through coordination and collaboration. The partners will use the plan's findings to guide the design and delivery of health services in the region.
