New Jersey Health Initiatives: Collaborating for Community Needs Assessment--Saint Peter's Foundation

The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.Saint Peter's University Hospital and Robert Wood Johnson University Hospital will collaborate with other community agencies, primarily in Middlesex and Somerset Counties, New Jersey, in a project entitled: Hospitals Collaborating to Assess and Address Changing Community Health Needs. This grant will engage the community through a multi-pronged strategy to collect quantitative and qualitative data. Methodologies will include: focus groups; community forums; key informant interviews; partnering with county health departments and community-based organizations; and use of technology such as the internet and social networking. The project will identify priority health needs and, for each of the identified health needs, will develop, along with community input, a detailed implementation plan that includes short- and long-term goals and measurable objectives that are realistic and achievable. Expected outcomes include: (1) a better understanding of community health needs; (2) a firm knowledge of the scope of existing resources; (3) more targeted services addressing the top priority needs identified by the community; and (4) better coordination of services.
