The purpose of this project is for the Foundation to partner with other funders to support the newly formed National Council on Aging's Self-Care Management Alliance (the Alliance). The Alliance's purpose is to promote strategic collaboration among government, business, and nonprofit organizations to embed evidence-based, self-care management programs in delivery systems for health and social services. Self-care management is the active participation of people in their own health care and includes the development of problem-solving skills, improved self-efficacy, and patients applying their new knowledge and skills in day-to-day management of their chronic conditions. Deliverables will include a white paper that synthesizes evidence-based self-care management programs and metrics; development of a plan to evaluate the effectiveness of various self-care management interventions and the capacity of delivery systems of health and social services to implement them; exploration of strategies to increase access to and sustain community-based and online self-care-management programs; establishment of a standing panel of consumers to provide input to the Alliance; development of a campaign to engender self-care management awareness; and development of specific Alliance goals and objectives for 2020 and strategies to achieve them.
