Improving transitions in care: Our Lady of Lourdes Health Foundation's Transitional Excellence Through Coordinated Systems

The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.Our Lady of Lourdes Health Foundation, Inc., will fund the Transitional Excellence through Coordinated Systems (TECS) that will adopt the teachings of evidence-based models such as Project RED and the Transitional Care Model within Our Lady of Lourdes Medical Center in Camden, New Jersey, to manage and promote effective transitions in care for adult populations with chronic health conditions and complex social issues. The two-year project will serve a cohort of 1,450 patients. Through the use of a coordinated system, and direct navigation from a Discharge Advocate RN, the TECS project will result in improved quality of care; reduced costs and reduce inappropriate use of hospital services. Partners under the TECS project include: Our Lady of Lourdes Medical Center; Lourdes Medical Associates; Lourdes Wellness Center; LIFE at Lourdes; Bayada Nurses and Compassionate Care Hospice. Comprised of two hospitals, Our Lady of Lourdes Medical Center in Camden and Lourdes Medical Center of Burlington County in Willingboro, and multiple outpatient services, Lourdes Health System can fully support the complexities of a care transitions project, particularly within a complex urban environment such as Camden, New Jersey.
