The Foundation's initiative, Mobilizing Action Toward Community Health (MATCH), was designed to help communities translate the County Health Rankings into multisector action that addresses the social, economic, environmental and behavioral factors that affect health.Under this grant, United Way Worldwide (UWW) will increase awareness of the County Health Rankings among local United Way affiliates and create a learning infrastructure to help affiliates use the rankings to engage multisector community partners to address the social, economic and environmental factors that influence health. By providing targeted training and technical assistance, fostering peer learning, and disseminating tools and case studies, UWW intends to help affiliates integrate the Mobilizing Action Toward Community Health (MATCH) model with their business practices, including the signature workforce campaigns and community-engagement activities related to health, income and education. In addition to integrating the rankings and MATCH models with existing UWW trainings and leadership forums, deliverables for this grant will include: webinar trainings; virtual toolkits; case studies and stories that highlight innovation and best practices among the affiliates; a virtual platform for peer-to-peer learning; and a comprehensive review and analysis of federal, state and local resources available to address the multiple determinants of health. Additionally, UWW will establish the High Performance Group for Health learning laboratory to test and share strategies on how affiliates can best integrate the MATCH model with their work to address health, income and education in communities.
