This grant supports a transitional phase for Improving Performance in Practice (IPIP), a technical assistance provider that has worked with a subset of Aligning Forces for Quality (AF4Q) communities in ambulatory quality improvement since 2004. Through AF4Q, 17 communities across the country are improving health care through alliances of those who get care, give care and pay for care. In addition to direct funding to the alliances, AF4Q provides technical assistance to support their work in implementing the primary AF4Q goals: public reporting of health care data, quality improvement and consumer engagement.
During this transitional phase, IPIP will develop its existing tools and resources into a new and more sustainable model of technical assistance that will enable its methods of ambulatory quality improvement to reach a far greater audience of ambulatory care providers across all AF4Q communities. The key deliverable for this six-month grant will be a sustainable technical assistance model for ambulatory quality improvement that includes a user-friendly and responsive menu of comprehensive tools and resources and minimizes the need for on-site assistance.
