Extending the national study of small- and medium-sized physician practices to three additional communities

The Foundation's initiative, Aligning Forces for Quality: The Regional Market Place, was designed to help communities across the country align health care forces to improve the quality of health care.This project will survey a representative sample of small physician practices in up to three new Aligning Forces for Quality (AF4Q) sites about their structure, the processes they use to ensure the quality of preventive care and care for chronic diseases, their external incentives to improve quality, and a select number of processes and practice characteristics that address racial and ethnic disparities in care. The grantee will use data collected in this survey as baseline data for an AF4Q evaluation. Deliverables will include the dataset and reports to the physician practices that participated in the survey. The Robert Wood Johnson Foundation will also include the dataset in its Health and Medical Care Archive.
