The Foundation's program, Healthy Kids, Healthy Communities: Supporting Community Action to Prevent Childhood Obesity, was designed to catalyze and support policy and environmental change projects to promote children's physical activity and healthy eating, especially in lower-income and racial/ethnic communities at greatest risk.The purpose of this project is to reduce obesity rates in three low-income communities through a grassroots planning process in which community members identify needed policy and environmental changes and leverage resources to promote their own healthy eating and active living. Deliverables include: developing and maintaining a diverse leadership team, conducting Food Store and Physical Activity Surveys to determine food availability and physical activity opportunities within each community, implementing a healthy snack program in after-school programs serving partnering communities, and developing a healthy eating and active living environmental and/or policy initiative within each partnering community based on identified needs.
