Development fund: strengthening Oregon's ability to deliver patient-centered care

The Foundation's initiative, Aligning Forces for Quality: The Regional Market Place (AF4Q), was designed to help communities across the country align health care forces to improve the quality of health care.This grant provides funding to the Oregon Health Care Quality Corporation to assist a cadre of patients and health care organizations in implementing structures for meaningful patient input at all organizational levels. The project will: (1) assess current patient-centered care among local Quality Corp partners; (2) share the current state of patient-centered care and set the stage for improvement; (3) recruit six pilot sites to commit to transforming their organization; (4) provide technical assistance to the pilot sites and share the lessons with community partners; and (5) celebrate improvements and learnings. The deliverable from this project is a replicable model of patient centered care with a comprehensive set of lessons learned and tools for other AF4Q communities.
