The Foundation's program, Aligning Forces for Quality: The Regional Market Project, was designed to support grants and technical assistance to community coalitions to work toward high-quality, patient-centered and equitable care.The Foundation is seeking to sustain high quality patient-centered and equitable care in a set of target regions by 2015. Under this strategic objective the Foundation has already launched the Aligning Forces for Quality (AF4Q) program directed at 15 communities and their respective alliance leadership teams. This program focuses on three critical areas: performance measurement and public reporting, consumer engagement and quality improvement. With this grant, the Foundation has asked the George Washington University to assume a leadership role as the AF4Q national program office (NPO) to manage six major activities: (1) national program management; (2) focused technical assistance to the alliances; (3) technical assistance to providers and health plans; (4) research and information; (5) legal barriers and policy; and (6) communications.
