The Foundation's Aligning Forces for Quality (AFQ) initiative seeks to improve the quality of health care in up to 20 regions across the country. These regions will require baseline data on quality and disparities to help them improve and sustain patient-centered, equitable care. The Foundation is committed to providing targeted regions with data and technical assistance that will support the translation of data analysis into local action. This project will support Drexel University in analyzing national, state, and local data relevant to AFQ communities. The goal of the project is to create comprehensive data inventory profile reports for eight regions. The Foundation's Quality/Equality Team will then use the reports to develop selection criteria which will aid in making decisions about which new communities to invite to apply to participate in the AFQ initiative. Each profile for the eight newly identified regions will include regional population characteristics, the context of care, health status indicators, financial ratios and individual-level hospital quality and performance indicators. An accompanying regional narrative will be included and will highlight disparities, nurse-sensitive care measures and nurse workforce information. The project will be considered successful if it results in: (1) eight data profiles on regions identified by the Quality/Equality Team, and (2) narrative summaries of the regions and a detailed appendix describing the data analysis methods used.
