Robert Wood Johnson Foundation Community Health Leaders

The Robert Wood Johnson Foundation Community Health Leaders (CHL) program was designed to provide recognition for the contributions community health leaders make to achieving RWJF's mission and goals and to enhance the capacity of these individuals to have a more permanent and widespread impact on health care problems.The CHL annually honors 10 individuals who have made significant contributions to improving health and health care in their communities. Each Community Health Leader receives an award in the amount of $125,000. Twenty thousand dollars goes directly to the leader to recognize past accomplishments and $105,000 goes to the leader's organization to support the continuation and enhancement of the leader's work. This grant supports the Omaha Tribe of Nebraska, the organizational home for the 2007 Community Health Leader. The Omaha Tribe of Nebraska will serve as the fiscal agent for the project, and the ongoing project activities will take place at the Carl T. Curtis Health Education Center.The purpose of this project is to establish an infrastructure that will enable the Carl T. Curtis Health Education Center to increase its federal Medicaid matching funds to increase the overall income for this much needed community health center. The Center provides an array of primary care, dental, and vision services. It also houses a fully functioning dialysis unit. This will be accomplished through training in the Medicaid Administrative Match (MAM) program which uses federal Medicaid matching funds to provide reimbursement to community-based programs for administrative activities related to outreach and coordination for people who are potentially eligible for Medicaid or current recipients.
