The Foundation's program, Project HealthDesign: Rethinking the Power and Potential of Personal Health Records, was designed to expand a vision of personal health records and encourage the market to develop products that meet the diverse needs of patients. This project will use Tablet PCs to reduce re-hospitalization via improved transition care across care settings by incorporating a bar code scanner. The project hopes to capture medication taken by a patient since scanning in medication information is easier than re-writing it. A further reaching goal is to allow for interoperability with other physiological monitoring devices to further increase use and data collection. It is expected that using the Tablet PC, in concert with the personal health record, will reduce medication errors, increase adherence to prescribed regimens and, therefore, reduce hospitalizations.
