Technical assistance and direction for RWJF's New Health Partnerships program
The purpose of this project is to develop and pilot test innovations in the health care delivery system that promote collaboration among providers, patients and families in the self-management of chronic illness. This will be done through several on-site and virtual quality improvement collaboratives and development of a virtual patient/family community.
