Multifaceted approach to reducing medication errors

The Institute of Medicine (IOM) report, To Err is Human: Building a Safer Health System, identified medication errors as the most common type of error in health care and attributed several thousand deaths to medication-related events. A 2006 IOM report, Preventing Medication Errors, estimates that medication errors harm at least 1.5 million people every year. The extra costs of treating medication-related injuries occurring in hospitals alone conservatively amount to $3.5 billion a year. It is widely asserted, though not empirically demonstrated, that the quality of nursing care provided in an institution is directly related to the number of injuries or deaths attributed to medication errors. This project will quantify nurses' role in medication management and produce new information that can improve nurses' ability to prevent harm from medication errors. This project is part of a broader medication management initiative (MMI) that, if successful, will significantly accelerate current progress towards eliminating harm from medication errors for U.S. patients through research and convening activities. The project will fill an important gap by producing at least one measure that can be used to assess quality in an area identified by the National Quality Forum and others to be a priority.
