Development of national rapid learning systems

This project builds on the work done under a phase-one grant that researched potential national strategies for Rapid Learning systems. These systems were conceived as real-time reporting of clinical experience for millions of patients to national databases for collaborative research. Such information is seen as important for quality, patient safety, effectiveness of new technologies, and policy decisions. In the first phase, the project team helped to generate strong interest in national Rapid Learning systems and their development. The project's major conference, with Health Affairs, produced six papers (plus seven commentaries) that will appear in a special web edition of Health Affairs in January 2007. The project also assisted an Institute of Medicine (IOM) workshop on the Learning HealthCare System in July 2006. The IOM's report, with short papers from 35 workshop presenters, will be published this year as well. The papers for the Health Affairs issue and the IOM report illustrate that Rapid Learning strategies are now starting to engage national health policy and research discussions. Under phase 2 supported by this grant, the project will advance the development of Rapid Learning systems in three key areas: (1) a national system of Rapid Learning networks; (2) a core capability for advancing clinical care for Medicaid enrollees; and (3) a national initiative for comparative effectiveness research with a specific focus on the Medicare population. The deliverables expected under this project will be: (1) a series of on-going meetings with experts and stakeholders along with briefings to key audiences (e.g., Congress, CMS) to advance the systems development work in each of the three key areas; (2) three national conferences in partnership with Kaiser Permanente and the University of Massachusetts; (3) a series of papers authored by the project team as well as commissioned work; and (4) a second Rapid Learning issue of Health Affairs. The project will be considered successful if it leads to the actual design and development of one or more Rapid Learning systems as well as results in policy recommendations for promoting such systems.
