Low-income residents of Washington, D.C., suffer from an unusually high prevalence of several diseases; this situation is exacerbated by the city's historic geographic and economic divisions. To address these issues, One Economy with the help of this grant support will implement a project, called "DC Health Care for You," which will take advantage of recent technology opportunities, including municipal wireless efforts, online health care applications and content, to help isolated low-income people manage their own health. The project is a collaborative effort with the Foundation's Pioneer team. The project allows the Foundation to determine the health benefits that result from investments in wide-scale patient self-management tools. This grant supports One Economy in: (1) collaborating with residents with in-home access and establishing partnerships with health-focused stakeholders to determine the scope of online content to be developed and securing support; (2) launching a bilingual, multimedia-rich Web site; (3) developing and implementing a comprehensive marketing and outreach plan; and (4) measuring the project's success in changing the behavior of the target population.
