Linking bloodstream infection rates to intensive care nursing context of care and process

The Foundation's Interdisciplinary Nursing Quality Research Initiative is designed to provide funding for interdisciplinary studies that address critical gaps in what is known about nursing quality and for the synthesis, translation, and dissemination of results to key stakeholders.This nurse-led interdisciplinary research team will refine an intervention proven to reduce central line-associated blood stream infections to better use the expertise of nurses and develop a measure of quality that reflects the positive contributions of nurses to the quality of care provided in hospitals. The team will conduct a randomized clinical trial in 84 hospitals to evaluate the intervention's impact on patient outcomes and nursing turnover. By focusing on a nurse-directed intervention that targets a narrowly defined and very significant adverse event, the investigators will be able to make strong assertions about the influence of nursing on quality. This study is likely to inform other nurse-led medical error reduction interventions and will contribute significantly to the quality improvement literature and to the science of nursing measurement. Additionally, this project should produce at least one nursing-sensitive measure that can be considered for endorsement by the National Quality Forum. A unique contribution of this study is the effort to empirically link nursing care processes to patient outcomes. Importantly, this team will examine in detail the effects of the context of care (e.g., the nurse work environment) to the success of the intervention. Finally, the project will identify barriers and facilitators to implementation of this intervention.
