Engaging health centers' participation in the national Academic Chronic Care Collaborative

The Chronic Care Model has been successfully implemented in hundreds of clinical settings but has been adopted in relatively few academic ambulatory clinical practice settings. This grant provides support for key components of the national Academic Chronic Care Collaborative that have been developed as a partnership between the Association of American Medical Colleges (AAMC) Institute for Improving Clinical Care and the Foundation's Improving Chronic Illness Care program. This Collaborative will engage 22 academic health centers that are investing in improving the care of chronically ill persons who receive ambulatory care in academic settings. It will also enable medical and nursing trainees, as upcoming stewards of the health care system, and their faculty to learn in clinical settings that reflect the best achievable care for chronically ill patients. The Collaborative breaks new ground in academic settings by providing a model that will bring expertise in health care improvement, reliability, safety, and collaborative care with patients to these complex environments. Key deliverables include the adoption of the Chronic Care Model in 22 academic health centers, the identification of new AAMC standards for medical education in the field of ambulatory chronic care, and the dissemination of tools and findings to implement these standards.
