The Foundation's initiative, Prescription for Health: Promoting Healthy Behaviors in Primary Care Research Networks, was designed to develop, field test, and disseminate innovative and feasible interventions for primary-care based health behavior change counseling.Practice systems to promote healthy behaviors must combine five attributes to be effective and sustainable. Patients benefit when primary care practices are equipped to address multiple health behaviors and provide comprehensive assistance as envisioned by the "5A" framework: assess, advise, agree, assist, and arrange.The purpose of this project is to use the Chronic Care Model as part of a system for health behavior counseling in practices to connect to patients with the intensive assistance that sustains lifestyle changes and enables clinicians to leverage simple measures outside of their practices and disseminate those measures for widespread adoption and substantial population health benefits.
