Studying the use of an interactive e-health portal to improve chronic disease care among adult primary care patients

The Foundation's program, Health e-Technologies: Assessing New Tools for Chronic Disease Management and Health Behavior Change, was designed to catalyze the development of more efficacious interactive health programs on health behavior change and disease management by evaluating promising interventions and providing information resources and tools for professionals.The purpose of this project is to use a patient Internet portal-based intervention to improve the care of adult primary care patients with three chronic conditions--pain, depression, and difficulty walking. To address this goal, the project team will conduct a randomized, controlled trial of an intervention to screen 5,000 patients at 5 adult primary care practices for the conditions using PatientSite, a Web portal serving more than 20,000 patients in eastern Massachusetts.
