The Foundation's Advancing Diabetes Self-Management program was designed to expand and test multicomponent, self-management programs that could be delivered in primary care settings and improve outcomes and cost effectiveness.Self-management and community supports are among the six elements the Chronic Care Model identifies as essential for a health care system that encourages high-quality chronic disease care. Though the benefits of self-management have been documented in numerous efficacy trials, the feasibility of promoting self-management and community supports in real-world settings is not well documented. This grant provides support for an evaluation that will systematically analyze the implementation and outcomes of 14 diabetes quality improvement demonstrations supported by the Diabetes Initiative. This evaluation will provide a better understanding of the core components of effective patient self-management interventions and the barriers and facilitators for their implementation. Results will inform the Foundation's Quality Team's strategy to improve the quality of care provided to Americans living with diabetes and other chronic illnesses. This evaluation will be considered successful if it identifies promising options for improving self-management skills and important clinical outcomes for disadvantaged populations. Products of this evaluation will be distributed through publication in peer-reviewed journals and presentations at major scientific meetings.
