Diabetes Collaborative Learning Network

The Foundation's Advancing Diabetes Self-Management program was designed to expand and test multicomponent, self-management programs that could be delivered in primary care settings and improve outcomes and cost effectiveness.This grant supports integrated technical assistance and direction to grantees of the Foundation's two-part Diabetes Initiative using the mechanism of the ongoing Collaborative Learning Network of grantees. The Advancing Diabetes Self-Management component of the Diabetes Initiative is designed to demonstrate that comprehensive models for diabetes self-management can be delivered in six primary care settings, significantly improve a patient's clinical and behavioral outcomes, and improve the person's quality of life. The related Building Community Supports for Diabetes Care program component has been designed to improve diabetes self-management and metabolic control through six innovative community-based programs. Both programs share a common National Program Office. The technical assistance provided through this office and the Collaborative Learning Network of grantees will help these two programs develop and test replicable models for improving diabetes care and self-management among patients in underserved minority and low-income populations. The technical assistance and direction for both program components will be integrated by a common understanding of the requirements for high-quality diabetes care and self-management grounded in broad social ecological theory and the Chronic Care Model.
