The Foundation's program, Communities in Charge: Financing and Delivering Health Care to the Uninsured, was designed for local communities interested in improving access to care for low-income, uninsured individuals by rethinking the organization and financing of local care delivery.Under this program contract, the National Program Office (NPO) will host the fourth and final project directors' meeting, support communities' planned transition to self-sufficiency, carry out limited dissemination activities, work with RWJF to identify possible future dissemination strategies, implement transition strategies, and conduct ongoing NPO functions.
