The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.This grant provides support to the Brotherhood Health Initiative, a unique intervention that utilizes intensive case management, targeted outreach, and prevention services to decrease the health disparities that exist among young minority males in the city of Newark. Successful treatment of health-related problems for young minority males (ages 12 to 25) is complicated by the community's social and environmental context. The Brotherhood Health Initiative is based on Division of Adolescent and Young Adult Medicine's successful experience with HIV-infected youth, using case management services to effectively mitigate these environmental and social barriers. This system will include intensive case management, group support, and client advocacy services.
