The Foundation's initiative, Prescription for Health: Promoting Healthy Behaviors in Primary Care Research Networks, was designed to develop, field test, and disseminate innovative and feasible interventions for primary-care based health behavior change counseling.The purpose of this project is to answer three questions: (1) How are primary care practice improvements implemented? (2) Which practice improvements can be implemented successfully during the grant period? (3) Which features of primary care settings make practice improvements more likely? In addition, the program will provide support to grantees to test whether the practice improvements lead to patient behavior changes. The evaluation will answer these questions by: (1) documenting how practice-based research networks implement and test their projects; (2) providing key support to grantees on research design and data collection; and (3) collecting crosscutting information on the primary care practices that participate in the projects.
