The Foundation's initiative, Prescription for Health: Promoting Healthy Behaviors in Primary Care Research Networks, was designed to develop, field test, and disseminate innovative and feasible interventions for primary-care based health behavior change counseling.This grant provides support for a quality assurance nurse within each health system to be trained to provide consultation service that assists the practice in making practice system improvements to increase preventive service delivery to its patients. The main outcome measure is an overall increase in the use of health behavior assessment, counseling, referral, and follow-up through the use of a 5 A's assessment--ask the patient about the behavior, advise toward a healthy behavior, assess interest in changing behavior, assist with making behavior change, and arrange for follow-up.
