Developing a comprehensive, community-oriented system-of-care model
The Foundation's program, Community Partnerships for Older Adults, was designed to foster efforts of local public-private partnerships to improve long-term care and supportive services systems for older adults.This grant will assist the community partnership in developing strategic plans to improve the systems delivering long-term care and supportive services. The local partnership of public, private and non-profit long-term care and supportive services providers and interest groups, older adults, family caregivers, civic leaders, and others will: (1) assess the strengths and deficits of the community's existing long-term care and supportive services systems; (2) identify areas for improvement and determine which of these should receive priority attention; (3) develop a strategy to bring about desired changes in existing systems; (4) strengthen the partnerships among public, nonprofit, and private organizations, and (5) regularly convene a leadership group committed to coordinating a community-wide initiative to generate local solutions that will improve the organization and delivery of long-term care and supportive services for vulnerable older adults.
