The Foundation's Local Initiative Funding Partners Program is a matching grants program designed to establish partnerships between the RWJF and local grant makers in support of innovative, community-based projects that improve health and health care for underserved and vulnerable populations.Using outreach workers and a warehouse of supplies, the Client Advocate Program provides badly needed nonmedical and nonreimbursable services and equipment for 1,600 cancer patients annually. The purpose of this project is to address gaps in cancer care and support services required immediately after diagnosis, before or after skilled nursing, or prior to hospice services. This project will help local hospitals and established organizations fulfill otherwise unmet needs such as nutrition supplements, wigs, counseling, and transportation.
