The Foundation's Building Community Supports for Diabetes Care program was designed to develop innovative community support programs to strengthen the capacity of minority populations to adopt diabetes self-management skills and other lifestyle changes.This grant provides technical assistance and direction support to the National Program Office (NPO) for the Building Community Supports for Diabetes Care program. The NPO will: (1) solicit, review, and recommend up to six sites for the self-management program and up to eight sites for the community support program to the Foundation for planning grants to be awarded in the fall of 2002; (2) monitor and assess the efforts of the grantees during the planning year, provide support for local evaluation efforts, and develop the groundwork for the evaluation of the programs; (3) develop a strategic communications plan for the NPO and assist funded community sites with specific communications activities; and (4) provide technical assistance to coalitions and community-based efforts.
