Evaluation of RWJF's Community Partnerships for Older Adults program

The Foundation's program, Community Partnerships for Older Adults (CPOA), was designed to foster efforts of local public-private partnerships to improve long-term care and supportive services systems for older adults. The purpose of this project is to plan and implement the first five years of an evaluation that will provide both formative information and measures of outcome for this program. This project will include baseline survey work and case studies. The first deliverable will be a community-specific survey that will inform the grantees about unmet community long-term care needs. There will be additional products on an annual basis from case studies, document review, and interviews. These reports will concentrate on outcomes of interest to the Foundation and CPOA's National Program Office, and particularly track innovations. The first phase of this project will be considered successful if it results in an evaluation that includes useful information on innovations in community long-term care systems for the Foundation, the funded communities, and the field at large.
