Providing crisis intervention for families experiencing a medical problem

The purpose of this project is to provide temporary support to families and individuals suffering a medical crisis in order to prevent further financial and emotional trauma and medical deterioration. Each client will be assigned a case manager and will be eligible to receive short-term health and social services, as well as financial support. The project will case-manage up to 25 clients for up to 2 years, and address shorter-term needs of an undetermined number of additional clients. This project will be considered successful if the organization can establish and maintain a revolving fund for the purpose of providing such services.
