While the use of evidence-based medicine (EBM) and continuous quality improvement (CQI) has been tested extensively within the mainstream health care industry, certain delivery systems, including safety net and Indian tribal clinics, have not benefited from the diffusion of these processes. The purpose of this project is to pilot an EBM-CQI model of care for diabetes, asthma, and heart disease at one tribal clinic in Wisconsin. Through provider education and training on clinical pathways, using data collection and analysis, chart audits and patient feedback, patient care and community education will aim to improve the health of the target population. The project will be considered successful if, through education of staff and piloted CQI efforts, the clinic successfully adapts these models. If successful, the program can be a model for other tribal clinics in the state and nationally.
