Improvement of chronic disease management by restructuring provider payments
Under previous grant support from the Foundation, this research team found that 9.3 percent of an under-65 population with 1 of 100 chronic conditions accounted for half of all health care expenditures. The purpose of this project is to plan a major demonstration of an innovative case rate payment and quality measurement system for chronic disease at the five University of California medical schools in collaboration with a health plan or the University of California (as an employer purchaser).
