Technical assistance and direction for RWJF's Community Partnerships for Older Adults program

The Foundation's program, Community Partnerships for Older Adults, was designed to foster efforts of local public-private partnerships to improve long-term care and supportive services systems for older adults.This grant provides support for technical assistance and direction for this Foundation program. The purpose of this project is to assist communities in building comprehensive long-term care systems that include a range of social and health services to support vulnerable older adults and help them age successfully.
