Developing health teams for the Kinship Support Network

The number of households headed by nonparent caregivers of young children is growing rapidly in America, especially among grandparents assuming responsibility for rearing their childrens' children. This grant provides support for the development of health teams as a prime component of a model family service system targeted on these households. Through state legislation, the model is being replicated widely in California under the leadership of the Edgewood Center for Children and Families, San Francisco. Known as the Kindship Support Network (KSN), the model was originated by Edgewood, and provides case management to enrolled families that features home visiting by mature paraprofessional women from similar racial and ethnic backgrounds. Under this grant, the KSN model at the Edgewood Center and at current and future replication sites would be strengthened by the inclusion of a health team. This would be a new component of KSN sites comprising expertise in nursing care, child development, and clinical nutrition. The chief measures of project success would be: (1) institutionalizing Edgewood's capacity as a technical assistance and training center for KSN health teams; and (2) the development of community assessment tools and operating processes (including manuals) for replicating and sustaining the teams at scale.
