Coming Home: Affordable Assisted Living

The Foundation's Coming Home: Affordable Assisted Living Program, run through the NCB Development Corporation, was designed to undertake the development of rural community-based systems of chronic care. The purpose of this project is to create affordable models of assisted living targeted to low-income seniors and linked with existing community health care systems. The grantee will work with local nonprofit sponsors to create a demonstration model of affordable senior housing with services. They will: (1) select up to four demonstration projects, develop a second assisted living model that does not trigger licensure and allows providers a choice of approaches and funding; (2) build resources to attain perpetual affordability; and (3) build knowledge and expertise within the affordable housing and service communities.
