The Foundation's program, New Jersey Health Initiatives, was designed to support projects in New Jersey that develop new approaches to resolving the state's health care needs, focusing on the Foundation's goal areas.The purpose of this project is for the Teleheart: Congestive Heart Failure (CHF) Management Program to expand disease management services to residents of congregate senior housing in Bergen and Passaic Counties through the use of videophones. CHF patients will continue to receive visiting nurses, with greatly increased contact with nurses via the telemonitoring. Chronically ill elderly will be able to remain in their own homes while controlling the progression of their disease. The program is intended to decrease hospitalizations and overall health care costs to patients, providers and payers. If successful, Valley Home Care plans to expand the prototype to manage other chronic conditions.
