Improving the quality and capacity of safety net organizations to deliver chronic care

There are significant gaps between what is known and practiced in clinical care, including safety net provider settings. Under the Improving Chronic Illness Care (ICIC) Program, the Institute for Healthcare Improvement has successfully adapted its Breakthrough Series (BTS) approach to generate dramatic chronic care quality improvements in safety net providers. The purpose of this project is to adapt the BTS to distance learning techniques so that it can cost effectively reach virtually all Federally Qualified Health Centers and rural hospitals. The revised BTS will rely on the Health Resources and Services Administration's (HRSA) technical assistance network that they built to support the original ICIC Program BTSs. This project will produce an implementation plan that other foundations, RWJF, and HRSA could support.
