The purpose of this project is to develop an exploratory policy paper on how the standard fee-for-service Medicare program can relate to community-based social services, particularly for chronically ill beneficiaries. Many Medicare HMOs identify high-risk elders, intervene with case management and related tools, or refer the person to community-based services. By contrast, those actions do not happen in Medicare fee-for-service. Funds will be used to support research to complete a policy paper on this topic.
